How do I go about setting up a dental office osha manual?
OSHA publication 3186-06R (2003) practically does it for you. You have to put your name in the blanks as you copy it into your own manual, but it is very complete.
Check www.osha.gov for information on obtaining this little manual. They probably have a downloadable version, since it's less than 30 pages.
If you are in or near New Jersey, post another question, because I can HIGHLY recommend a lecturer for you. Post, "Can anyone suggest an OSHA lecture for dental offices in the New Jersey area?" and I'll answer you.

Reply:i tried to do it for my office. unless you set these things up routinely, there is no way you can do it on your own. we hired someone to do it for us and now everything is tight as a drum. it's totally worth it to hire it out because one violation can cost over $10,000 and i promise you that the osha agent is going to find more than one violation. it can be as simple as having one piece of paper in the wrong notebook or not having a chemical spill bucket.
let me repeat, this is not a job for someone to get on the job training. this must be hired out or you might as well skip the whole process and work on saving up enough money to pay the fines.
